Job Overview
We are seeking a skilled and compassionate Medical Social Worker to join our multidisciplinary healthcare team in Abuja. The successful candidate will be responsible for evaluating patients’ psychosocial needs, providing essential support, and helping to enhance patients’ overall well-being. Key Responsibilities
- Psychosocial Assessment & Care: Conduct thorough assessments of patients’ psychosocial conditions to identify their needs and collaborate with the healthcare team to address them.
- Collaboration & Communication: Partner with physicians, mental health professionals, and other healthcare staff to understand the social, emotional, and ethical issues impacting patients.
- Care Planning: Develop personalized care plans in consultation with the Medical and Clinical Directors to align with patients’ needs and recovery goals.
- Documentation & Reporting: Maintain accurate and timely documentation of patient assessments, interventions, and progress reports in line with professional and organizational standards.
- Support & Advocacy: Provide counseling and support to patients and their families, helping them build healthy coping strategies. Advocate on behalf of patients to resolve conflicts and improve their care experience.
- Crisis Intervention: Respond to crisis situations promptly, offering the necessary support and resources to stabilize patients’ mental and emotional well-being.
- Continuous Patient Support: Monitor patient progress throughout the diagnosis, treatment, and recovery phases, making recommendations for lifestyle adjustments, support groups, or other interventions as needed.
- Team Collaboration: Actively participate in multidisciplinary meetings to ensure coordinated patient care and effective discharge planning.
- Training & Development: Engage in ongoing professional development through in-service training and performance improvement activities.
Qualifications
- Education: Bachelor’s degree in Social Work, Social Work Administration, or a related field.
- Experience: At least 2-3 years of experience in a similar role, particularly within a healthcare or mental health setting.
- Skills:
- Strong interpersonal and communication abilities
- Proficiency in psychosocial assessment and care planning
- Ability to handle sensitive information confidentially
- Crisis intervention skills
- Language: Fluency in English is required.
Additional Requirements
- Relocation: Applicants must either reside in Abuja or be willing to relocate before starting work.
- Education Level: Higher National Diploma (HND) is preferred.
- Experience: Minimum of 2 years of relevant experience is required.
- Commute: Ability to reliably commute to the job location.
